Total Care Services is seeking a highly organized and experienced Coordinator of Community Services (CCS) to provide case management services to individuals with intellectual and/or developmental disabilities in Southern and Central Maryland.

*Eligibility to work from home after working at the Lanham, MD office for 30-90 days, and after showing your consistency with billing 28 billable units daily into the LTSS System.
Coordinator of Community Services (CCS) Representatives supports individuals with leading full lives. This assistance includes identifying, linking, and coordinating a variety of supports and services within their community. Each individual is supported in developing individualized care plans that will be reflective of their strengths, needs, respect, dignity, personal goals, choices, and values. All services that are provided are based on the principle of preferences, needs, desires, and goals when assisting the individuals in becoming fully integrated into the community, and working towards optimal independence.
Three (3) strong years of case management experience working with individuals with intellectual and/or developmental disabilities;
Three (3) strong years of managing a caseload of 40 (or higher) individuals with intellectual and/or developmental disabilities;
Extensive knowledge about Developmental Disabilities Administration (DDA) policies, procedures, service delivery systems, community resources, and Medicaid;
Bachelor's degree or higher in the Human Services field;
LTSS System and Targeted Case Management experience is highly preferred;
Ability to identify your caseload's needs during all monitoring visits, phone calls, and other interactions with the individual and/or his/her family;
Ability to establish and maintain working relationships, respond to individuals in a timely manner and use independent judgment and initiative to access services;
Ability to facilitate the empowerment of individuals through promoting independence, self-determination, and creativity;
Must bill 28 billable units per day and log notes into the LTSS System daily.
