Bergen’s Promise is the Bergen County based non-profit organization responsible for face-to-face care management and comprehensive service planning for children and their families with intense and complex needs. The youth we serve are challenged by serious behavioral emotional, mental health, substance abuse issues and developmental disabilities. We provide skilled care management to children and their families utilizing both traditional mental health services and creative community opportunities available to Bergen County residents.
We are currently seeking Care Managers to join our winning team. The Care Manager position requires care coordination and crisis work with children and families in Bergen County, NJ. Flexible hours. Must be passionate, creative and able to work under supervision and as part of a team, as well as be culturally competent/responsive.
This position DOES NOT provide clinical hours.
Follow the Wraparound model in all interaction with children and families.
Work with assigned number of youth to meet state and agency contract requirements and adhere to “No eject, no reject" policy.
Receive and review referral and collateral information prior to and in preparation for the initial child family team meeting.
Collaborate with the Family Service Organization to initiate Parent Partner’s attendance to “meet and greet."
Ensure that documentation within the electronic record and hard chart is completed, timely and accurate.
Care Manager’s shall adhere to the agency code of ethics within their education, work experience and discipline in addition to applicable professional boards.
Provide crisis coverage for all Care Management Organization families when scheduled to do so; following agency on-call protocol.
Maintain regular contact with service providers to obtain feedback and guidance with regards to the Individual Service Plan and updates within the Individual Service Plan.
Meet with the Care Manager Supervisor on a regular basis for consultation and supervision.
Identify new community resources and barriers, and ensure this information is conveyed to the Community Resource Manager.
Adhere to HIPAA standards and maintain client confidentiality.
Master's Degree in a related field (Social Work, Counseling, Psychology, Criminal Justice) preferred, Bachelor's Degree in Social Work or related field will be considered.
Minimum of one (1) year related experience (Care Management) preferably with at-risk children and families required.
Experience working with the intellectually or developmentally disabled a plus.
Culurally competent and responsive.
Be able to work under supervision and as part of a community team.
Experience with community relations, resource development, and Wraparound process are a plus.
Be energetic and creative.
Excellent communication skills.
Valid Driver's License is required.
Bi-lingual skills a plus.
