The Loudoun County Department of Mental Health, Substance Abuse, and Developmental Services (MHSADS) is seeking a smart, resourceful, and efficient clinical professional to join the Discharge Planning Team.
Provides the active, effective link between psychiatric hospital and community-based levels of care.
Works intensively with department and hospital staff to optimize communication and treatment for children, adolescents, and adults who are affected by serious mental health, substance use, and developmental disorders.
Collaborates with individuals, families, and department and community partners to create person-centered aftercare plans which support the recovery of the individuals and their families.
DCP services are mandated and require close attention to detail, strong organizational and interpersonal skills, and effective teamwork. This position offers many and frequent opportunities to “make a difference”!

Bachelor’s Degree in a related field required; and
3 years of paid work experience in mental health, substance use, and/or developmental disability services required.
Master’s Degree in a related field
Licensed or license eligible in the Commonwealth of Virginia
Bilingual in English/Spanish. Computer skills, e.g., Outlook, Word, Excel.
Employment is contingent upon successful completion of a criminal background check, credit check, driving record check and TB test. Must have a valid driver’s license in Virginia or regional state of residence.
